Evening Questionnaire

The Evening Questionnaire is a way of collecting information about a patient's daily activities in order to measure their physical, social and mental wellbeing on a daily basis.

How it works

The patient answers three simple questions about their day. From within the module, patients can view their progress in a graph and view their previous results in a table. Patients can also set a daily, weekly, or monthly reminder to help stay on track.

In the Huma Portal, care teams will see the latest Evening Questionnaire score for their patients, with any concerning scores flagged for attention. In the Patient Summary, care teams will be able to see all historical data in graph or table form.
